POSITION: Home Visitor (Family Support Specialist), Bilingual preferred.

The Home Visitor is assigned to provide comprehensive services to pregnant mothers, their
children up to three years old, and family through home visits and group socialization activities
in accordance with HFA Best Practice Standards and the Family Foundations Home Visiting
Program grant. To act as an Ambassador to the Agency and to reflect all Agency matters in a
positive way.
FLSA STATUS: Full-time, Non-exempt, 52 weeks/year
ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Ensures all HFA Best Practice Standards are met within the Family Foundations Home
    Visiting Program.
  • Responsible for initiating and maintaining regular and long-term (up to five years)
    contact/support with families.
  • Conduct visits to young children and their families, which will occur primarily within the
    family’s home; each visit should last for a least one hour.
  • Provide interventions that are family-centered, strength-based, and directed at:
    establishing a trusting relationship; assisting in strengthening the parent-child
    relationship; assisting parents in improving their skills to optimize the home
    environment; improving the family support system; and increasing the family’s ability to
    problem solve and assume the role of advocate for themselves and their children.
  • Identify and refer for contact/appointments at other supportive agencies including
    health care appointments, crisis intervention.
  • Responsible for assisting the family in establishing goals and a plan for accomplishments
    of those goals.
  • Use Growing Great Curriculum for child development in the context of the home and
    during group “socialization” experiences.
  • Facilitate group “socialization” experiences and other family group activities.
  • Conduct screening and ongoing assessment of normal growth and development of the
    focus child. Share screening and ongoing assessment information with families.
  • Promote parental involvement in curriculum planning, program advisory board, and the
    overall HFA program.
  • Monitor and provide education to pregnant mothers with respect to prenatal visits,
    children’s health status, including medical follow-up, physical health, dental health,
    mental health, and nutritional intake.
  • Provide management, crisis intervention, and resource referral services.
  • Maintain documentation in DAISEY and spreadsheets as required by FFHV and HFA.
  • Maintain confidentiality of family records and information.
  • Participate fully in supervision meetings and training experiences.
    CAPACITIES:
  • Home Visitors must have knowledge of infant and child development. Must be skilled in
    communicating, establishing trusting relationships, and accepting individual differences.
    In addition, they must have knowledge of community resources and the skills to link
    families with appropriate agencies and services.
  • Capacity to problem solve, handle crises, and willingness to work with the culturally
    diverse populations present among the target population.
  • Approach to working with families that is empathic, nonjudgmental, respectful, and
    professional.
  • Flexibility concerning time and days able to work and work tasks.
  • Must be open to reflective practice.

    QUALIFICATIONS:

    EDUCATION and/or EXPERIENCE:
  • A minimum of a high school diploma or equivalent and knowledge of maternal and
    infant health, parenting education, and concepts of child abuse and neglect, is required.
  • Associate degree in Early Childhood, Human Services, or related field, preferred.
  • College coursework in Early Childhood Education, Human Services, or a related field is a
    plus.
  • Experience working with families or providing services to children and their families is a
    plus. Familiar with the Sawyer, Rusk, Taylor, Washburn & Clark County communities
    including available resources.
  • Knowledge or experience with Healthy Families America is preferred but not required.
  • Fully Bi-lingual in Spanish/English, preferred.
    LANGUAGE SKILLS:
  • Demonstrates clear and effective English oral and written communication skills,
    appropriate respect, and good listening skills.
  • Fully Bilingual in English/Spanish, preferred.
  • Maintain confidentiality and professionalism of all family, volunteer, and staff
    information. Communicates optimistically, positively, and with honesty and openness.

    CERTIFICATES, LICENSES, REGISTRATIONS:
    Valid driver’s license with current vehicle insurance.

    COMPENSATION: $18 – $22 per hour.
