Dear Community Members,
Indianhead Community Action Agency, Inc. is a private, non-profit charitable organization which is governed by the Community Services Block Grant and Head Start performance standards which require us to conduct a comprehensive community needs assessment. It is VERY IMPORTANT to hear from our community members in the six counties we serve ( Rusk, Taylor, Clark, Sawyer, Washburn, and Burnett) on poverty and health related issues that affect you, your family, and the community in which you live, work, and play. We also welcome input from members who are from other counties and would ask that you identify the county you reside in should you be willing to participate in the survey.

Please take time to fill out this survey because your responses provides data that is used to show our rural community needs and supports funding and other grant related sources to support initiatives to address the issues of rural poverty and the subsequent health inequities and disparities.
